Team Forms

Notice: Effective 2024-25, Membership # no longer exists.
You can ignore it on all forms. If your school has more than one membership, make sure you include “Team A”, “Team B”, etc. on your forms AND your membership sign.
This is the list of all forms that the teams need to bring at tournament. The TEAM makes all the decisions related to the judging forms. For Primary and Division I, the Coach can help type and make copies but the TEAM decides how to fill them in.
MEDICAL RELEASE FORM for TX: One form only per TEAM MEMBER, who is present at the tournament. This is shown at the Team Check-In Table. The coach and every chaperone KEEPS this form AT ALL TIMES in case of an emergency. Give copies to all adults who will be responsible for the team on competition day. (Some schools and school districts will require that you also have a copy of the child's insurance card and may require additional medical release forms. Contact your school coordinator for details.)
STYLE FORM (PDF or DOCX): Four (4) copies are needed. Follow the instructions in your current long-term problem (section F) for style items 1, 2 and 5. Make sure that the "Free choice" options are replaced by your choice of style elements that you wish to be judged on. Be sure to specify WHICH item or costume the team chooses to be scored on. Do not include as a style element, any item already scored in long term (section B). A TEAM member gives this form to the staging area judge.
COST FORM (PDF or XLSX): One (1) copy only. A TEAM member gives this form to the staging area judge.
OUTSIDE ASSISTANCE FORM: One (1) copy only signed by all team members. A TEAM member gives this form to the staging area judge.
TEAM REQUIRED LIST FORMS: Four (4) Copies Needed. Select your problem's specific form under the red heading Team List Forms. A TEAM member gives this form to the staging area judge.
TEAM CONTRACT (optional in Texas, but recommended): Some coaches, schools, or school districts will require their team members to sign this contract. You do not need to present your team contracts at TX tournaments. If attending World Finals, each team member must sign this contract.
Important Note: Keep a copy of all forms. If you advance to the next level, you will need them again as-is or at least for reference. If you change your solution, you are expected to adjust your team forms for the next level.
